Admission Policy
Residents considered for acceptance into the home will be professionally recommended for community placement. Their primary diagnosis will need to be such that they could benefit from the Divine Haven Homecare and AFH program. As part of our admission policy and to ensure the residents are compatible, an assessment will be completed prior to admission that identifies the individual’s needs, abilities, interests, and expectations. This assessment will include both a personal interview and a record review including medical and social histories. A prospective resident’s health will need to be screened by a health care professional within 90 days prior to admission. The findings of this health examination need to be provided in a written statement and indicate that the prospective resident is free of communicable diseases. Candidates that appear to be compatible with others will be admitted. An admissions agreement will be completed prior to admission and signed by the resident or the resident’s guardian if that person’s consent is legally required.
Specific program goals include:
- Supervising or assisting a resident with or teaching a resident about activities of daily living.
- Providing, arranging, transporting, or accompanying a resident to leisure and recreational activities, employment, and other activities identified in the resident’s individual service plan (ISP).
- Providing, arranging, transporting, or accompanying a resident to medical or other appointments as part of the resident’s individual service plan (ISP).
- Keeping a current record of all medical visits, reports, and recommendations for a resident.
- Monitoring resident health by observing and documenting changes in each resident’s health and referring a resident to health care providers when necessary.
- Notifying the placing agency, if any, and guardian, if any, of any significant changes in a resident’s medical condition, including any life-threatening, disabling, or serious illness, any illness lasting more than 3 days, an injury sustained by the resident, medical treatment needed by the resident or the resident’s absence from the home for more than 24 hours.
