Position: Program Coordinator
Reports to: Director
Position Type: Full Time, Exempt
Office Location: Madison, WI

The Program Coordinator position is responsible for assisting the residents through the activities of daily living according to the resident’s assessment and care plan. This includes engaging with residents, providing, and assisting diverse activities appropriately while promoting as much resident independence as possible. The Program Coordinator will function as part of the interdisciplinary team and is able to work independently. The Program Coordinator maintains a caring attitude towards residents ensuring a safe and clean environment. Additionally, the Program Coordinator is responsible for setting a positive can-do attitude for every shift and supporting and providing team members in being successful in completing their daily job responsibilities.
The Program Coordinator is a caregiver who demonstrates exceptional leadership qualities on a day-to-day basis. He/she will support the caregiving team and the Director. He/she will have good interpersonal skills and be able to give positive feedback to all team members – motivating and inspiring team members to rise to a higher level.

Essential Duties and Responsibilities of a Program Coordinator:
  • Demonstrate good judgment that is in the residents’ best interest when it comes to assisting with activities of daily living & decision-making
  • Participate in management meetings, resident meeting, in-service meeting, and training as needed
  • On-call responsibilities as outlined in the On-Call policy
  • Participate in family events, marketing, and promotional events in the home
  • Provide team members opportunities to learn and grow
  • Train and mentor new team members in the New Team Member Onboarding program
  • Ensure compliance and participate in training as scheduled; support and encourage team members to do the same
  • Address caregiving concerns and questions from other team members
  • Review caregiving duties of team members to ensure completion
  • Provide tours to prospective residents or their families when the Director is not available
  • Follow the inquiry/tour protocol and efficiently and effectively complete and submit all paperwork and notify the correct individual that an inquiry or tour has occurred
  • Bring concerns regarding team members or residents to the attention of the Director
  • Assist with all ADL’s to provide the highest quality of life and care, including, but not limited to: bathing, dressing, grooming, incontinence care, vital signs, and weights
  • Monitor and record health and behavior of residents and communicate such information with other management and staff
  • Perform medication passing and record-keeping duties if assigned (must be certified)
  • Maintain the confidentiality of resident and client records, including identity, behavior, and medical conditions, and be cautious of discussing resident issues in front of others’
  • Supervise and monitor residents with regards to resident safety, following applicable resident care policy procedures and standards
  • Read and implement residents’ individual care plan
  • Familiarize self with the Policy and Procedure Manual; be prepared to implement its contents and/ or support the Director and leadership team with the implementation of its contents
  • Assist in household and maintenance duties on a daily basis, including, cleaning, laundry, inventory and storage of supplies, and outdoor maintenance
  • Use all materials, food, and supplies resourcefully; report needed items to the Director or shift Team Leader
  • Other duties as assigned

  • CBRF Certifications (preferred)
Education
  • Associate/Bachelors (Preferred)
License

  • Fire Safety (Preferred)
